For your convenience you will now find the forms necessary for admission. We will need a current medical history (not older than 5 days) and chest X-ray results taken within the last 90 days of admission.

Your patient will be required to provide the following upon entrance to Thornapple Manor. These include:

• Power of Attorney Paperwork
• Advance Directives
• Living Will
• Insurance Cards
• Social Security Cards
• Guardianship Paperwork
• Ambulance Card
• Five to Seven Clothing Changes
  • Make sure they are washer safe
  • We will mark them for you with permanent laundry markers
• Skid Proof Shoes
• Calling Card for Long Distance Calls
